Ok explaining this should be easy c:

The first box is for the voice actor
Name and picture

The second box is for the character they've portrayed (if they haven't done a character- eg a singer, whatever you can have this blank, doodle there, another picture, whatever.) (but a lot of time even singers will have played roles in movies or tv shows and stuff) 

The last box is where you draw your character :3
